Background and Activities

The NHPCC New England Region was formed in 2012 by the merger of CDC Regions I and II for the MCHB grant. The original Region II includes 14 centers in New York, New Jersey, and Puerto Rico, which provide CDC Community Counts surveillance data. Regional NHPCC-supported activities include:

  • Updating transition of care (TOC) surveys for patients
  • Participating in Waves I and II of the NHPCC’s quality improvement (QI) pilot project
  • Developing multiple educational brochures for consumers
