NHPCC Communities of Practice

The NHPCC has established a number of communities of practice (CoPs) designed to thoughtfully foster discussion and idea-sharing among HTC staff across the nation.

CoPs bring people together from across the nation by discipline or by topic to promote collaborative and interactive learning opportunities that:

  • Promote professional development and idea-sharing for promising new practices
  • Focus on the organic learning process that evolves as CoP members learn about each other’s work
  • Meet at specified, regular intervals (i.e., quarterly)
  • Include a resource-sharing component

Current NHPCC CoPs

  • Advanced Practice Providers – started May 2023
  • Aging – starting Fall 2025 (Open to any HTC staff who work with adult patients)
  • Nursing – starting Fall 2025 (Open to all HTC nurses)
  • Operations – starting Fall 2025 (Open to any HTC staff who work on clinic operations, such as maximizing efficiency with appointment times, reducing no-shows, creative ideas around no-shows, space constraints, creative ideas for establishing satellites/outreach clinics, etc)
  • Physical Therapists – started January 2024
  • Social Workers – started May 2023
  • Quality Improvement – started June 2023

How to join an NHPCC Community of Practice (CoP):

  • If you are an APP, SW, or PT, you will be added to that CoP distribution list & calendar invites. If you have not received those calendar invitations, please reach out to us at nhpcc@athn.org
  • If you are on staff at an HTC and working on or interested in quality improvement, please complete this interest form, if you’d like to join the NHPCC Quality Improvement CoP.

“Communities of practice are groups of people who share a concern or a passion for something they do and learn how to do it better as they interact regularly… People in communities of practice share their experiences and knowledge in free-flowing, creative ways that foster new approaches to problems.”
